Kelton Vasileff, MD Orthopedic Surgeon Ohio State Wexner Medical Center] W. Kelton Vasileff, MD: Gluteal tendon tears can certainly be treated without surgery. One of the first things we try is a period of relative rest and avoiding activities that incite and make the pain worse. After that, we progress with physical therapy formally with our physical therapists, as well as some home exercise programs. Beyond that, we can progress to corticosteroid injections, platelet-rich plasma injections, shockwave treatment, and Tenex treatments if we need to do those things before we progress to a surgery.
