Position summary

PCRM - Rotation (Job Opening 423302) - Scope of Position/Position Summary:
The Patient Care Resource Manager (PCRM) is a nurse case manager who navigates patients through the health care system to include admission through post discharge coordination of care, resources and services utilizing the nursing process. The PCRM monitors and facilitates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization. The PCRM identifies and removes barriers to progression of the plan of care for each patient. The PCRM collaborates with physicians, nurses and other health care workers to manage patient transitions to the next level of care. The PCRM is the central information source for third party payers, communicating the plan of care, medical necessity, and gaining approval for post discharge care. The PCRM, through the use of evidence-based criteria, assists the physicians in ensuring the appropriate level of care for each patient. The PCRM maintains an awareness of state and national health care trends and regulations including TJC and the Centers for Medicare/Medicaid Services and adjusts practice accordingly.

Education and experience

Bachelors degree required, Masters preferred. RN with 3-5 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior oncology nursing or case management experience preferred. Must be able to work with various computer applications.
