Position summary

Social Worker (Job Opening 436002) - Social Worker provides social work services to patients of the Hemostasis and Thrombosis Center (HTC) as well as collaborates in the development of goals and administration of programs provided through the HTC; conducts psycho/social assessments of individual and family adjustment to chronic and acute disease (bleeding disorder and its complications such as HIV, Hepatitis C, hemarthroses) to include counseling related to medical condition(s) and treatment(s), the individual?s acceptance/integration of and coping with their disease, adherence level, and barrier reduction; provides counseling and psychotherapy, crisis intervention, individual, marital, family and group therapy; assesses patient and family knowledge and understanding about their bleeding disorder, its complications (HIV, hepatitis, hemarthroses), genetics, treatment recommendations, and available resources ; provides relevant education related to diagnosis, treatment, and interpreting medical recommendations; collaborates with medical director and nurses on the development of program goals; provides input and ideas on program development and evaluation of program progress; reviews and ensures adherence to CDC/MCHB and Cascade grant guidelines; participates in the writing of grants; completes budget and required reports; consults on program needs for CDC and MCHB grants; provides outreach to underserved; collaborates and consults with other hemophilia center staff both in the state and nationally; advocates within medical center, with external providers, community agencies, legislators, insurance companies for the needs of bleeding disorder patients.

Education and experience

Master of Social Work from an accredited Graduate School of Social Work required; State of Ohio Social Work license (LSW or LISW) required; experience in a healthcare or medically related setting required. Requires successful completion of a background check; selected candidate may be asked to complete a pre-employment physical, including a drug screen.
