Specialty Practice Pharmacist (Job Opening 452137) - Scope of Position:
Acts as a resource and provides answers to drug information questions for patients, families, and other health care practitioners. Approves new drug orders, checks/verifies chemotherapy, and processes non-formulary requests.  Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions.  Participates in appropriate hospital and/or departmental committees relevant to practice area.

Position Summary:
This position coordinates all aspects of patients' pharmaceutical needs. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for appropriateness.    Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age.  This position also supervises Pharm.D. students, technicians, residents, and provides educational programs for hospital staff.

Duties and Responsibilities:
Dispenses medication pursuant to a physician's order according to the laws and regulations as defined by the Ohio State Board of Pharmacy and institutional policies.  Enters or reviews entry of medication orders into the pharmacy computer system and ensures accuracy of dispensing. Fulfills all applicable MUE requirements. Complies with OBRA 90, HIPAA requirements and accreditation standards applicable to area of practice.

Education and experience

Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred (e.g. PGY1 and PGY2 residency).
