Licensed Pract Nurse-Pharmclgy (Job Opening 437888) - Scope of Position
The LPN is a licensed practical nurse working under the direction of the RN case manager to function within the multidisciplinary team as an advocate, educator and facilitator of resources for patients to ensure the appropriate access to medical services under the Patient-Centered Medical Home model of care.

Position Summary
The LPN receives direction and support from the members of the practice team to provide evidence-based patient-centered care to patients. The LPN may assist with the following duties under the direction of the RN Care Coordinator, Physician or Nurse Practitioner (NP): (1) assisting patients with access to appropriate care and services, promoting self-care management and prevention; (2) assisting with identification and removal of barriers to care and coordinating patient care services by utilizing the nursing process and care plan created by the RN Care Coordinator to help patients move through the healthcare continuum; (3) assisting patients with the previously developed plan of care in accordance with nationally recognized evidence-based standards; (4) assisting in education under the plan of care by instructing patients and their caregivers in understanding and managing their diagnoses, treatment, and symptoms.
Goals should include: Supporting the healthcare team in maximizing quality chronic care management, enhancing the patient experience, avoiding preventable admissions and readmissions, avoiding preventable/non-emergent ED visits, and increasing the delivery of recommended preventive services.

Education and experience

Licensed Practical Nurse with current Ohio licensure, current BCLS and NAPNES certification.  Minimum 1 year experience in a physician's office or other primary health care setting preferred.
