Ohio State offers comprehensive, research-driven care developed specifically for older adults with cancer
The Ohio State University Wexner Medical Center’s OrthoCONNECT program is a collaboration between its orthopaedic and geriatric medicine teams, designed to enhance care for older patients who have received musculoskeletal care. Both departments have been nationally recognized for excellence by U.S. News & World Report, which makes this collaboration all the more powerful.
Seniors who sustain an injury face many barriers on their road to recovery. Lack of transportation to follow-up appointments, confusion about medications and breakdowns in communication often lead to medical setbacks. Our mobile outreach program provides a unique bridge to older patients by providing on-site services.
OrthoCONNECT addresses common barriers to care by providing:
- Phone consultations
- Acute injury on-site visits
- Procedural visits (cortisone injections for arthritis, splint or cast management for fractures)
- Postoperative visits
The partnership has filled troublesome gaps in care – leading to better outcomes and a more seamless continuum of care for older orthopaedic patients. The Ohio State Wexner Medical Center OrthoCONNECT program is in its infancy but has resulted in substantial improvements in patient outcomes.
Since its initiation in April 2018, OrthoCONNECT has resulted in:
- Superior patient and family satisfaction ratings
- Reduced complications
- Fewer hospital readmissions, specifically surrounding wound healing
