Ohio State offers comprehensive, research-driven care developed specifically for older adults with cancer
Over the past five years, Ohio State has partnered with Heartland of Dublin and Heartland of Westerville to provide subacute, transitional, long-term and hospice care.
Both locations provide full medical care and rehabilitation services. Patients benefit from state-of-the-art rehab facilities that offer physical, occupational and speech therapies to help them regain important life skills that increase their independence and quality of life.
The Ohio State Subacute Nursing Program brings together a multidisciplinary team of experts who deliver nationally recognized care. The team consists of:
- Board certified geriatricians
- Dual certified geriatricians and hospice and palliative medicine physicians
- Full-time geriatric nurse practitioner
- Geriatric fellows
- Resident physicians in internal medicine and family medicine
- Physiatry consultants
The goal of the program is to help patients recover and regain independence skills after an illness or injury. In addition to medical care and rehabilitative services, patients are educated about their conditions and how to prevent setbacks or recurrences. Patients of this program are often recovering from joint replacement surgery, diabetes complications, heart conditions, stroke or cancer. Together with inpatient consultants, home health nurses and hospice nurses, Ohio State Wexner Medical Center works to facilitate smooth transitions from the hospital back into the community.
