University Reference Laboratories (URL) provides the high quality laboratory testing and the expertise of the Ohio State University Wexner Medical Center Clinical Laboratories to community health care institutions and professionals throughout Ohio and beyond. Shipping
All specimens submitted to the laboratories must have accurate, complete and legible patient identification on the specimen container, whether primary or aliquot. Identification of the specimen must include both the patient's full legal name and identification number.
For safety purposes rather than glass, whenever available PLASTIC SPECIMEN CONTAINERS are to be used. All specimens must be submitted in a rigid, puncture resistant, sealed container.
In accordance with OSHA safety regulations, all primary specimen containers must be leak proof and placed in a secondary leak proof container for transport to the laboratory. Securely sealed biohazard specimen transport bags are used for this purpose. Requisition forms are to be placed in the outer pocket.
Shipper's responsibilities include identifying, classifying, packaging and marking/labeling hazardous materials or dangerous goods according to all national and international governmental regulations.
