• How do I prepare for my visit?

  Have this information on hand when you register:

  • Your driver’s license or state-issued identification
  • Insurance card
  • Referral from MD, DO, NP, DC which can be printed and brought to appointment or faxed to 614-685-1808
  • Emergency contact information
  • If prescheduled, appointment date and time
  • List of current medications and medical history, including previous surgeries

  *When looking at your insurance card, there are a few specific pieces of information you will be asked for.

  First, your member ID number, which is shown on the bottom right example. Second, your eligibility/claims number, which is shown on the bottom left:

  

• For students, are appointments free of charge?

  The PT Clinic through Ohio State may have a charge associated with it. As a patient, it is your responsibility to ensure you are covered for any services you receive and to provide accurate insurance information to facilitate the billing process.

  If you do not have insurance or are under-insured, you can learn more about Financial Assistance through Ohio State’s Wexner Medical Center. For specific benefit and coverage questions, please contact your insurance carrier directly at the number on your insurance card.

Ohio State Wexner Medical Center offers a variety of tools on MyChart to get estimates on out-of-pocket estimates. For questions about your specific insurance coverage, you need to call your insurance company.

  MyChart is a free and secure online portal to exchange messages with your provider, request appointments, renew prescriptions, check your test results and much more. We will be able to assist you in signing up for MyChart during your visit.
