Ohio State Sports Medicine Rehabilitation is proud to partner with Denison University to bring physical therapy services into the Granville area. The Denison University Sports Medicine Rehabilitation clinic is open to everyone.
Physical therapy services include:
- Sports related injuries such as sprains, strains and return to play
- Post-surgery return to activity
- Keeping active with osteoarthritis
- Spine ailments such as neck and low back pain

- Your driver’s license or state-issued identification
- Insurance card
- Referral from MD, DO, NP, DC which can be printed and brought to appointment or faxed to 614-685-1808

- If prescheduled, appointment date and time
- List of current medications and medical history, including previous surgeries
*When looking at your insurance card, there are a few specific pieces of information you will be asked for.
