The care you receive at The Ohio State University Wexner Medical Center is documented on your Electronic Medical Record (EMR), which you may hear referred to as your Epic or IHIS chart. You may request information from your medical record at any time.
How to Request Your Medical Records
There are three ways to request information from your medical record regarding your care at Ohio State.
- Download and complete the medical records authorization form and return it to the appropriate address indicated on the form.
- For deceased patient requests, download and complete this form.
- Request information directly from your MyChart account by selecting "Send us a Customer Service Request" under the Request My Record section.
- MyChart is a free and secure online portal to exchange messages with your provider, request appointments, renew prescriptions, check your test results and much more. The Ohio State MyHealth app makes MyChart accessible from your smartphone or tablet.

The Ohio State University Wexner Medical Center’s inpatient tablet, MyChart Bedside is available to manage your health care needs online.
How to get a tablet
Ask your care team to set you up with MyChart Bedside. We can provide you with a tablet during your stay, or if you prefer, you can use your own mobile device. You do not need to have a MyChart account to use MyChart Bedside.

- Watch patient education videos and read more about your health concerns
- Communicate with family members
- Communicate with your care team using messages or video
- Order your meals and submit other “I would like” requests

After you are discharged from the hospital, continue to manage your health care from home with a MyChart account. Our patient portal, MyChart provides easy and safe access to your health information online.
In addition to signing up and accessing your MyChart account, Ohio State’s mobile app, Ohio State MyHealth, provides additional tools to help make your experience with us simpler.

During your stay, loved ones can access your inpatient information and stay up to date using the MyChart Bedside tablet you are provided. If you do not have a MyChart account and would like to provide a family member access to your inpatient information who is not in the hospital with you, you will need to create a MyChart account and provide them proxy access. You can sign up for MyChart using the MyChart Bedside tablet or the website.
To add a health care proxy, log in to MyChart and search “sharing” from the main menu and select the proxy access sharing option that meets your needs. Once you add a health care proxy, your loved one or caregiver will have the ability to view your health information or communicate with your health care provider.
