The care you receive at The Ohio State University Wexner Medical Center is documented on your Electronic Medical Record (EMR), which you may hear referred to as your Epic or IHIS chart. You may request information from your medical record or participate in your care through MyChart, which provides a portion of your overall medical record and offers communication services with your providers’ offices.
Medical Records Requests
To request information from your medical record regarding your care at Ohio State, download and complete the medical records authorization form and return it to the appropriate address indicated on the form.

The Ohio State Wexner Medical Center offers MyChart, a secure, online connection to your healthcare records. With MyChart, you can request appointments, renew prescriptions, review your health history and more.
- When you are discharged, you will receive an After Visit Summary that includes a MyChart activation letter. This document will contain your activation code and your Medical Record Number (MRN) needed to create your own MyChart account
- You may also talk to your Ohio State provider during your admission or at your next visit about signing up for MyChart
Once you have activated your MyChart account, you can download the free Ohio State MyHealth app from the Apple or Google Play app stores. The MyHealth app makes MyChart patient portal accessible from your smartphone or tablet.
